In the comment box on the second page (after clicking the submit button)

Please enter the following information:

Name of the person you want the certificate made out to.

Name of person giving the certificate.

Address you want us to ship the certificate to.

Please Note: Gift certificates are shipped with a postal signature confirmation. If the certificate does not show up it is likely sitting in the post office waiting for a signature.
